Who it Serves

Our Early Identification & Referral (EI&R) programs serve children from birth through age five. Some programs are targeted to children with suspected developmental delays.

Ages 0-3

Families with babies and toddlers

Families with preschool-aged children

How It Works

EI&R services include developmental screenings for children ages 0-5; referrals to appropriate resources and support services; and patient information including strategies to monitor growth and development.

Home Visiting

In-home coaching with trained parent educators
